Assisted Living
Our Assisted Living program is ideal for those who require regular assistance with daily activities, providing the perfect balance of independence and support.
Within Assisted Living, specific levels of care are tailored to each member’s needs, as determined by ongoing assessments. Levels include Essential, Intermediate to Enhanced and Comprehensive to Custom.
ESSENTIAL
Assisted Living members can expect personalized support with everyday tasks while still maintaining a strong sense of independence. This typically includes regular help with medication management, nutritious meals and light housekeeping, as needed.
INTERMEDIATE TO ENHANCED
For those requiring additional care, hands-on assistance with mobility, personal grooming and daily wellness routines is readily available.
COMPREHENSIVE TO CUSTOM
Communities provide ongoing supervision and health monitoring for members with higher needs, complemented by an on-site team and thoughtful coordination with medical professionals to ensure confidence, continuity and peace of mind.
Memory Care
Heirloom Memory Care is grounded in empathy, connection and human-centered research, creating a supportive environment that truly honors each person’s story, strengths and dignity. Our team meets members with Alzheimer’s disease and other forms of dementia where they are, adapting communication and care so they can experience success and comfort in each moment. We focus on lifting up their abilities rather than their losses, creating opportunities for engagement that are meaningful and tailored to who they are – not just to their diagnosis.
Heirloom Essential
For members with early-stage dementia who are experiencing memory loss or need guidance and routines, our programs enhance cognitive engagement, social interaction and well-being through music, art and light exercise.
Heirloom Enhanced
This level of care is specifically designed for Heirloom members who require additional hands-on assistance and support. Dedicated associates are available throughout the day to provide help, ensuring that members feel comfortable and safe.
Heirloom Custom
Our dedicated team provides compassionate support to residents who require continuous care, ensuring that their dignity and comfort are always prioritized. We understand the unique needs of each individual and strive to create a nurturing environment.
